Registration Form for New/Existing Lifts

Important Information
· A new lift is one that is made available to users/put in service from the 1st of July 2002 onwards whereas an existing lift is one that is made available to users/put in service before the 1st of July 2002.
· For the registration of a new lift/s, the Declaration of Conformity and the Details of the responsible person are to be attached. In this case, this form should be completed by the installer.
· For the registration of an existing lift/s, no attachments are required. In this case, this form should be completed by the responsible person.
· Payment can be affected either online, bank transfer or cheque payable to the MCCAA. In any case you opt for a bank transfer or cheque, a proof of payment needs to be attached.
  I, hereby declare that the details furnished in this application are true and correct to the best of my knowledge and belief and I undertake to inform the Malta Competition and Consumer Affairs Authority of any changes therein, immediately. In case any of the above information is found to be false, untrue, misleading or misrepresenting, I am aware that I may be held liable for it. I understand that this form will be considered invalid unless any additional documentation required, as specified in the notes above, is attached and duly filled.
  The Malta Competition and Consumer Affairs Authority ('the Controller') will process your personal data in accordance with the relevant provisions of the General Data Protection Regulation (GDPR) and the Data Protection Act (Chapter 586 of the Laws of Malta), for the purpose of lift registration, in accordance with the relevant provisions of the Lifts Regulations, Subsidiary Legislation 427.37 and Inspection of Lifts Regulations, Subsidiary Legislatoin 427.63 of the Laws of Malta. I am aware that this requires the disclosure of my name, Identification card number, home address, e-mail address and personal contact number to the Installer for the purpose of providing the required personal data to the Controller for lift registration purposes. The Controller will process your personal data for registration and administrative purposes and to comply with its legal obligations.
