Description: This is an application form for the approval of lifts that do not conform to Schedule I, Section 2.2, of the Lifts Regulations, LN 231. It only applies to those lifts that do not have the necessary height of pit and/or headroom prior to their installation.

Form Name: Application Form for the Authorisation of a Plant Protection Product (PPP) and Adjuvants
Description: This is an application form intended for a new authorisation of plant protection products via Mutual Recognition or adjuvants to be placed on the Maltese market. In this form it is required to provide information on the product, applicant, the marketing authorisation from other EU Member State competent authorities, information on the active ingredient/s, letter of access (if applicable), product classification, registration report (dossier), labelling and packaging. The applicant is also required to attach copies of the documents mentioned in the application form including proof of payment.

In this form it is required to provide information on the product and applicant, details of reference product, packaging, labelling and of product to be imported, confirmation of identicality such as original label and safety data sheets. The applicant is required to attach copies of the documents mentioned in the application form including proof of payment.

In this form it is required to provide information on the product and applicant, details of reference product, product to be imported, labelling, confirmation of identicality such as original label and other information including the area of agricultural land registered. Form Name: Application Form for the Authorization of Research and Development of a Plant Protection Product
Description: This is an application form intended for an authorisation of research and development of plant protection products. In this form it is required to provide general information on the applicant, details on the plant protection product including active ingredients and quantities to be used or released, dossier or a letter of access for the dossier, information of the research to be carried, contingency plan, safety data sheets and labelling. The applicant is required to attach copies of the documents mentioned in the application form including proof of payment.

Description: This is an application form intended for companies involved in maintenance and servicing of equipment utilising fluorinated greenhouse gases to register with MCCAA. Form Name: Application Form for the Zonal Authorisation of a Plant Protection Product
Description: This is an application form intended for zonal authorisations of plant protection products to be placed on the market including new products, label extensions, renewal of authorisation and change in conditions. In this form it is required to provide information on the product and applicant, type of application, evaluation relevance as well as all compulsory documentation. The applicant is also required to attach copies of the documents mentioned in the application form.
Description: This is an application form intended for the authorisation of a clone product of an authorised plant protection product. In this form it is required to provide details of the applicant, details of the clone product and of the reference product authorised in Malta and manufacturing sites of the clone product.
